| Description |
The shaft of the P302 Benfield section centrifugal pump had failed by fatigue. There were two fatigue cracks in the shaft and both of them had initiated at the sharp corner of the keyway edge. The most probable reason for the fatigue crack initiation appears to be misalignment of the shaft and/or loosely fitted key. No metallurgical and/or mechanical abnormalities were responsible for the failure.
