Description The shaft of the P301 Benfield section centrifugal pump has failed by fatigue. There were multiple fatigue crack initiations at the slot meant for fixing the split locking-ring. Evidences suggest that there were mechanical damages at the fatigue crack initiation sites. The mechanical damages were caused probably due to edge biting of the locking-ring on the shaft. The fatigue cracks had initiated at these stress raisers. The shaft material was found to conform to 17-4 PH steel and there were no metallurgical abnormalities in the material.
