Description The IGB failure was due to fatigue fracture of the driving gear. There were three major fatigue cracks and six fatigue crack origins. Failure pattern and damages on the teeth suggest that the fatigue crack initiation was due to excessive load on the gear. It appears that the extraordinary high load on the gear was due to misalignment in the gear train, which would have occurred during the assembly after the last overhaul of the IGB. No metallurgical and/or mechanical abnormalities in the gear were responsible for the fatigue crack initiation.
