Description Stress shielding is one of the major reasons for Total Hip Replacement (THR) failure in long term. The effect is more prominent at proximal femur and shaft portion, after femoral stem is inserted in the femur canal. This paper reviews several published research articles, in a few groups based on different design improvement approaches of femoral stem to minimize the stress shielding effect. It reveals that minimization of stress shielding in femur part is possible by varying fixation method and a few basic design parameters of stem like geometry, internal structure, surface roughness, material etc. However, till date the exact solution or range of values of above parameters could not be established, which can solve the stress shielding problem for entire population. The future research on femoral stem design should continue to provide range of optimum values for each design parameter based on geometry of femur and its bone condition to minimize stress shielding.
