Description Fractographic examination showed that eight blades out of nine blades have failed by fatigue. The ninth blade has failed by overload and this failure might have occurred subsequent to the primary fatigue failure in the other eight blades. Fractographic features on the fatigue fracture surfaces indicated unusual load spectra on the blades. Analysis suggests that the blades were subjected to high load peaks intermittently during the entire service life. It appears most probable that these high load pulses were responsible for the premature fatigue crack initiation in the blades. However, with the available evidences, it was not possible to establish the reason (s) for such load spectra on the blades. The blade material was found to conform to specification in terms of composition, microstructure and hardness. A detailed analysis of the failure has been presented in this report.
